Date (DD/MM/YY):
Time (24HR):
Destination:
Your Number:
Your Email
Notes:
And if possible of a way to prevent of spam being spent by adding a captcha
Does anyone how I can create it so it will post the above information entered will automatically sent to an email account?

I would strongly recommend using an established contract manager form with CAPCHA (although i'm afraid I can't suggest anything specific as I've not had this requirement myself) - as I wouldn't really be comfortable suggesting code for you that involved email delivery, as any kind of weakness in the system is open to abuse by spammers! Much better to use an established script that will have already been peer-reviewed for this kind of application...
